Action Plan Worksheet

Once you understand the process of goal-setting (as outlined in the “Get Up and Goal” workbook and guide), you’ll want to take specific steps toward achieving each of your goals. You can list those steps in the “information and skills” section of your journal or on a  “Goal Notes” sheet. However, sometimes steps will change throughout the process of reaching your goal and you may need more space to make changes. Use this action plan worksheet to help organize your steps. If your plan changes you can simply print a new sheet. Once you’ve created your action plan, post a copy of it in your room to view it daily.
